Medical Coverage for Parents & Parents-in-Law

As a Kaiser member, you may enroll your parents, parents-in-law, or parents of your domestic partner in a TPMG group plan
 

Physicians pay for parents' coverage through automatic payroll deduction

   

Benefit coverage includes comprehensive health care coverage for services provided at Kaiser Permanente Medical Centers with a $20.00 office visit co-payment

   

Prescription drugs covered with a $10.00 (generic) or $25.00 (brand name) per prescription co-payment

   
Emergency room visit (waived if admitted to hospital) covered with a $35.00 co-payment
   
Eligible to enroll within 60 days of hire; coverage begins on the first day of the month after enrollment form is received in the Regional Physician Benefits Department or upon CMS approval of Senior Advantage, if later. Once elected, coverage cannot change until the next Annual Open Enrollment unless parent experiences a qualifying event
   
Parents over age 65 wishing to enroll in Kaiser's Senior Advantage Program must assign their Medicate benefits to Kaiser. Other more costly options are available
   
Parents residing in Kaiser service areas outside of California may be eligible for enrollment in TPMG sponsored group contracts in Colorado, Georgia, Hawaii, Mid-Atlantic, Northwest and Ohio regions. Rates and benefit converage may vary; contact Regional Physician Benefits for more information and appropriate enrollment forms.
   
Should you leave The Medical Group, your parents' coverage is cancelled effective the end of the month in which you terminate from service. At that time, your parents may convert to a direct-pay plan through Kaiser Member Services

Dental Coverage for Parents & Parents-in-Law

Parents residing in California may elect to enroll in DeltaCare PMI dental coverage. They must choose a participating DeltaCare dentist

Cost is $15.91 per parent per month during 2008

   

If elected, coverage is effective the first of the month following the date the completed enrollment form is received by the Regional Physician Benefits Department

   

Once elected, coverage cannot change until the next Annual Open Enrollment

   
DeltaCare provides some services at no cost, while many services require co-payments. See DeltaCare brochure for details
   
This dental coverage does not provide the same Delta Dental benefits as provided by TPMG to physicians and their eligible dependents
