Overview” Sick Leave and Disability
Long Term Disability Coverage
Physician pays premium (voluntary participation)
Eligible at date of hire or upon reclassification from contract
to associate; must enroll within 60 days of hire date or reclassification
Coverage begins on the first of the month following receipt of the enrollment
form by the Regional Physician Benefits Department
You may elect a monthly benefit of either 50% (to a maximum benefit of $16,667) or 60% (to a maximum benefit of $20,000) of your covered prorated monthly base
(up to $33,333) compensation
Disability Benefits paid by the insurer are tax-free
Six months qualifying period before you are eligible for benefits (income continuance available during qualifying period)
Income continuance available only to those enrolled in LTD
Residual Disability "Work Incentive" Benefits: If you can work part-time at your regular occupation, or perform some work at any other occupation including limited or modified job duties or schedules on a full or part-time basis for less pay you may qualify for residual disability benefits under this plan
